What your receipt will show
Our logo and business details, your name and address, purchase date and event, each item with its price, sales tax, total paid, card type with last 4 digits, the FDA 510(k) number for EMS/TENS devices and pads, and the intended use: over-the-counter pain management and recovery.
Note: Some plans also require a Letter of Medical Necessity from your provider. Check with your plan administrator. Powered Living does not determine HSA/FSA eligibility.
